Practical information about the application process

For most types of visa applications, you will need a medical certificate. The Belgian Embassy in Berlin does not maintain a list of designated doctors. You can choose any qualified medical doctor in Germany to complete, stamp and sign the medical certificate in the specific format (see section forms and certificates to download).

If you need a legalized medical certificate to apply for a work permit, you should have any German medical doctor fill out the certificate. Once the medical certificate is filled out, you send it to the Belgian Embassy in Berlin to be stamped. Please include a self-addressed stamped envelope. There is no fee. The correct address is
